All Aadhar Operator / Supervisor,
Kindly Find the Attachment that's shows "Aadhar Operator Activation Form".
Another form of Gujarat Govt. fill up this form for every inactive/Fresh operators and Supervisors.
This is mandatory for all inactive and fresh Operators and Supervisors.
Fill up proper Govt Premises Address with pin code and sign & seal by District UID Nodal officer and District manager sign.
Than send scan copy via mail or Hard copy via courier.
